What is a care plan?
A care plan is a jointly agreed, written plan between you and your care co-ordinator or lead professional which outlines your assessed needs, any risks to yourself or others, personal goals and progress in your recovery.
Your care plan should…
- Include your personal goals towards recovery and self-care as well as treatment and intervention goals
- Include everyone that supports you and detail what that support is
- Be written in plain and simple language
- Include information on what to do in a mental health crisis and manage risks
- Include information on staying well and coping skills in times of difficulty
- Have contact details for local mental health services
Service users…. did you know?
- You can be involved in developing your care plan
- You can have a copy of your care plan
- You should be involved in the review of your care plan at least every 12 months or within 6 months for those on Care Programme Approach (CPA)
- Your care plan may include any Advanced Statements or Decisions you have written
- Your preferences about your care co-ordinator or lead professional will be considered and taken into account.
Family, carers and supporters….did you know?
- You can be involved in the care plan with the service users agreement
- Your views are important, should be considered and taken into account, even if they are different to those of the person you support
- You can arrange to talk on your own to a care co-ordinator, lead professional or named nurse
- You are entitled to a carers assessment in your own right
Staff….did you know?
- The care plan should be jointly developed with the service user
- Service users are entitled to a copy of their care plan and a copy must be offered
- Carers should be involved in the care plan with the service users agreement
- You should review the care plan at least every 12 months in collaboration with the service user or within 6 months for those on CPA
- You should get consent from the service user to share the care plan with everyone involved in their support and recovery
